Patient’s Payment Responsibilities:

All fees and charges associated with telehealth services are the patient's responsibility, including any co-pays or coinsurance, according to the insurance plan.

Providing payment information authorizes immediate charging of the account at the time of appointment confirmation, and for other additional services rendered after your visit.
​
The right to change or introduce a new pricing structure at any time is reserved.
​
Please note that your insurance may not pay for your telehealth visit and other services associated with your visits, it is your responsibility to check with your insurance to make sure that your visit will be covered.

You are responsible for paying 100% fee for all services provided to you if you do not have insurance.
All payment and fees are due on the day of service provided to you.

Additional charges may apply to other requested services during visits, including extended visit time, laboratory, x-ray, and others.
Insurance Coverage:
Accurate insurance information must be provided.

Failure to provide accurate information will result in responsibility for the full cost of the service, unless otherwise required by law.
It is important to verify coverage with the insurance plan, as some companies may not cover all telehealth visits. If no insurance information is provided, full payment is charged immediately on the account at the time of appointment confirmation, and for other additional services rendered after your visit.

If the insurance information provided and insurance benefit eligibility could not be determined, you authorize full payment to be charged immediately to the account at the time of appointment confirmation, and all other charges on the account after visit.
You are responsible for providing accurate credit card and bank card information. You are responsible for any returned payment authorization and all fees incurred Failure to pay for services rendered will be subject to debt collection or other means permissible by law.
Cancellation Policy:
A missed appointment fee of 75% may apply.
A cancellation fee of 50% may apply if a scheduled appointment is cancelled less than 48 hours prior to the day of appointment.
If your may cancel your appointment for any reason, you may do so 48 hours prior to the day of the appointment.
All payment is final, no refund to payment made for the services received.
No Guarantee of Coverage:
Telehealth services are offered for convenience and are not mandatory.

The choice to use telehealth will not impact the right to receive in-person care.
